Life Disability

Supplemental Employee Life

Full-Time Hourly employees have the option of 1-5x salary

Supplemental Spousal Life

Covered spouses can elect up to 50% of employee life amount, starting at $10,000

Short-Term Disability

All Team Members are eligible for 60% of base salary, up to $2,000 per week
Lower rates for hourly Team Members

Voluntary Accidental Death Dismemberment

Coverage options are available to match supplemental life (1-5x salary)
